All Digital Health Initiatives

Nova Scotia is undergoing several different digital healthcare projects, with One Person One Record being just one project of many. Listed below are a few examples of the current digital health projects being built within Nova Scotia. Each project is working in unison as part of a greater healthcare digital transformation.

Oncology Transformation Project (OTP)

The Oncology Transformation Project is a collaborative project with cancer care, industry and government. The goal is to build a single, seamless provincial oncology (clinical) information system, to enable the highest quality patient care and meet Cancer Care Program needs. When implemented, the solution will provide single-entry referral, triaging and scheduling of patients and real-time communications within and between all Cancer Care Program specialties and sites.

  • Streamline coordination of care and transition of care close to home or within the patient’s home.
  • Enable easy, real-time access to patient information to ensure the right care, at the right time, in the right place, by the right provider.
  • Optimize and expand Patient Reported Outcomes (PROs) collection to improve patient experience, engagement and outcomes.
  • Develop a robust data strategy to optimize use of resources and inform service delivery planning.
  • Improve patient safety and decrease wait times through standardized electronic processes.

Care Coordination Centre (C3)

C3 is the first province-wide, command centre model in Canadian health care. It will directly support the six solutions identified in Action for Health, the Government’s strategic plan for the health care system.

C3 is improving how we coordinate the movement of patients throughout their health care journey. It allows us to give Nova Scotians the best care possible by:

  • We need to improve access and the flow of patient care within our facilities. We have created a Care Coordination Centre, known as C3, that uses a command centre model to better coordinate decisions to improve patient experience and care delivery.
  • C3 supports teams to identify issues and find solutions using real-time data. This will translate into:
    • Improved patient access and flow
    • Efficient bed management and discharge planning
    • Better scheduling of staff
    • More collaboration between health zones across the province
  • C3 is generating actionable insights that will enable us to continuously improve how health care is delivered, to improve the quality of patient care.

eReferrals

As part of Nova Scotia Health’s Surgical Access and Quality Improvement Strategy, IWK Health’s Quality Improvement Plan, and government’s Action for Health Strategic Plan, electronic surgical referral (eReferrals) launched in March 2023 and the tool was expanded in the fall of 2023 to support magnetic resonance imaging and ultrasound eReferrals.

Primary care providers, surgeons and diagnostics imaging teams are currently using the tool to help better manage and coordinate referrals for these services and other services and providers are expected to be added over time.
